Report Hours of Service Violations & Failures in Route
The reporting page for failures in route has been expanded to include hours of service violations. Simply open the page, hit the three lines in the upper right corner, choose which form you need to complete, fill in the blanks and hit submit. All data is kept by this office and is not shared with the Carrier.
Thank you for your help in collecting this important information. We will be adding a section for interim breaks, 4 hour breaks, soon as we would like to see a change in that area.
