Proof of Vaccination Requirement

Meeting Location: Perry’s Pizza Joynt, 39 w. North Ave., Northlake, Illinois
Effective January 3rd, 2022 Cook County Department of Public Health has issued Order No.2021-11, COVID-19 Mitigation, dated 12/23/2021, which requires indoor dining establishments to verify that patrons are fully vaccinated. In order to enter Perry’s Pizza Joynt, you will be required to show valid proof of vaccination:
· A CDC COVID-19 Vaccination Record Card; or
· An official immunization record from the jurisdiction, state, or country where the vaccine was administered; or
· An ‘app’ that provides a digital record of vaccinations; or
· A digital or physical photo of such a card or record that includes the
individual’s name, the vaccine brand administered, and the date the
vaccination was administered.
