Employee Submission:
I am requesting the points assessed on my record be removed for the layoff on (date) at (time) as this is a violation of my right to a fair and impartial hearing under the Act. My layoff was for good and sufficient reason.
If you were denied a compensated day add:
I was denied the ability to use a compensated day by CMS due to a manpower shortage thereby forcing me to use a non-compensated layoff status.

Use this process to get points removed! I got assessed TEN (10) points on 03/08/2022 during my undisturbed rest. The caller I was talking while tying up pressed the wrong button and inadvertently put me back onto a "call list" and I miscalled for my assignment while in undisturbed rest. I created a ticket explaining this mishap and those points were removed in 30 minutes.
